Task 2: Describe the support needs for providing palliative and endof-life care

 In this task, you can choose a situation that you personally have experienced on the job OR you can use the scenario we’ve give you about a tangata whai ora receiving palliative and end-of-life care.

If you’re using your own example from the workplace, make sure you first describe the tangata whai ora for us (Note: please keep the name and identity of the tangata whai ora anonymous).

a. Describe the situation
• Identify the type and stage of dementia the tangata whai ora has
• Identify where they are receiving care (home, residential care or hospital care unit).
• Identify any other conditions that the tangata whai ora may have.
• Describe the physical, mental, social and spiritual needs that the tangata 
whai ora has that are caused by the dementia and other conditions.

b. Describe the care and support needs
Use your own example OR the scenario we’ve given you to describe the care and support needs of the tangata whai ora in palliative and end-of-life care.
• Identify two key principles which are relevant to this person’s palliative and end-of-life care.
• Describe how each of the two principles support the needs of the tangata whai ora
• Describe any other care and support needs you should be aware of when working with vulnerable clients
