Task 1: Components and strategies of personal plans
Read the personal plan of the tangata whai ora you have selected and then complete the following.
- Name three components of their personal plan (for example, personal care, medication support, routines, promoting independence, learning new skills, specific activities, or aspirational goals).
- Describe what the goal is for each of the three components of the personal plan.
- Describe the strategies used to meet the goals of each of the three components of their personal plan. Your answer must demonstrate how these are done in a person or whānau-centred and strengths-based way.
Task 2: Implementing the personal plan
Complete a log on the occasions that you work with the tangata whai ora. You must log the actions you take to help implement the personal plan goals you identified in Task 1.
You can use the tables provided below to log your answers, or you may choose to develop your own log. There must be a minimum of four entries. Whichever option you choose, your answer for each entry in the log must clearly include:
- the date and time of your involvement/action
- which component of the plan was being worked on
- what goal(s) was/were being addressed at that time
- the actions you took to meet the goal(s) of the personal plan. (This must be done in a person or whānau-centred way.)

In this task, you will evaluate progress toward the components and goals you discussed in Task 2. You will need to identify any changes that might be necessary as a result of your ongoing evaluation. For each goal explain the following things in writing.
- Progress towards the goal.
- What progress has been made toward the goal?
- How did you evaluate progress toward the goal in a person-centred way?
- Ways that you have recorded/reported progress toward the goal?
- Recommended changes to the goal and implementation plan.
- Any changes that might need to be made to the personal plan goals or implementation as a result of your ongoing evaluation.
Your actions must align with your organisation’s requirements, and your recordings must be written objectively and from a strengths-based perspective.
