How to Structure Your Assignment

1. Topic Selection and Literature Search

  • Select a topic from the list provided and conduct a literature search for
    current literature (within the last 10 years) related to the topic.
  • There should be a minimum of 10 references and should primarily include
    journal article(s) to support your discussion.

2.  Part A

Read the literature and:

a. Discuss what is generally known about the topic and how it relates to the long-term conditions/care context.

b. Examine why this is an important topic for long-term conditions/care
in the current healthcare environment, i.e., an over-burdened healthcare system that is set up for acute conditions and has a primarily bio-medical approach to care.

c. Demonstrate understanding of how the topic impacts people with a
Long-Term Condition (LTC), i.e., the lived experience of the person/patient.

  • Include patient examples.
  • Include patient quotes (from the literature).

3. Part B

Read the literature and:

a. Examine how the role of the Registered Nurse (RN) applies to the topic.

This should include discussion on:

  • What roles/positions RNs are working in
    (in regards to your topic), for example:

    • Case Manager
    • Specialist Nurse
    • Navigator
    • Generalist role applicable to your topic
  • What does the RN “do” in regards to your chosen topic.

    Note: Remember most of the time RNs are working with people with LTCs in a community setting.

  • What works well and what needs to develop to ensure excellent nursing care
    of people with LTCs (applicable to your topic).

Additional Instructions

  • Please use some of the journal articles/resources throughout the
    LTCD course on Northnet to help you with this assignment.
  • You will also need to do some literature searching yourself in order to complete the assignment.
  • You can also refer to the NorthTec Academic Template
    (found on the Long-Term Conditions & Disability course on NN)
    to gauge a good understanding of expectations of Level 6 writing.

Reference Requirements

  • References must be journal articles.
  • 20 references required (NZ-based references, please).
  • Double Line Spacing.

Aim: To facilitate the students’ understanding of the complexity of working with and caring for persons with a disability and/or chronic illness, and to develop skills and attitudes which support understanding of and promote effective nursing interventions for people with chronic illness and/or disability.

Assessment Criteria

  1. Conduct a search of the current literature related to ONE of the following topics:
    • Case management
  2. Demonstrate your understanding of the topic in the context of long-term care and the role of the Registered Nurse within this, by examining the literature.
  3. Formulate a 2000-word written assignment utilising valid current literature.
  4. Must achieve standard for Structure and Organisation.
  5. Must achieve Academic Standard.
