SCENARIO
Mrs K is an 89-year-old resident of an aged care facility. She has several co-morbidities and is quite frail, but is cognitively alert and able. She is prescribed seven regular medications. Registered Nurse (RN) CJ is on the morning shift and is doing the medication round with Anna, a Student Enrolled Nurse. Mrs K says she doesn’t want to take her medications today, and tells the nurses to take them away. She states that she is constipated but doesn’t want to take any medication for this either. She says, “I take too many pills, and they only make me feel worse, I want to use a traditional therapy made from natural ingredients which I have with me.” When they leave the room, RN CJ states to Anna that she thinks Mrs K. should have an aperient for the constipation. She proceeds to crush all her medications and mix them with Mrs K’s yoghurt. She states that, “Sometimes we have to do this with residents. This way, Mrs K. won’t know that she is getting them, and she must take them.” RN CJ asks Anna to go and give Mrs K. the yoghurt mixture while she moves on to the next resident, as she is running late. Anna has some doubts, but follows the RN’s instructions. Later, on the evening shift, Mrs K. complains of dizziness and appears confused. Her heart rate is increased and a little irregular. Her blood pressure is lower than normal. In the morning Mike, the Nurse Manager, does an investigation. When asked, Anna mentions that she gave Mrs K crushed medication in yoghurt at the request of RN CJ. After further investigation, Mike finds that Mrs K has no prescription for the aperient on her medication chart. The administration of, or reason for, this medication has not been documented anywhere in the nursing notes of her patient file.
